Add a field on edit load? And Upload Multiple Files?

Is there a way to add a multi-line field when a user opens a record to edit. I would like to track history without letting a user edit what was previously entered, and date stamp it?

Also, I would like to have a file upload option, but where the user can click something like 'upload another file'

Can this be implemented in Creator?
