Allow users to add new entries: form doesn't reflect all "on load" actions

The new functionality is good, but the form that is brought up after clicking "Add a <form entry> " option doesn't reflect the actions for "on load." I created a form with a lookup that counts whether a user's login is listed  multiple times, and if not that particular field is hidden.  This works fine in the actual form, but not when initiated through clicking the "Add a <form entry>" option.
 
As an example: Support has access to the application: Final Meaningful Use Method.  The relevant form is brought up from any of the numbered views under tab Step 2 by clicking edit on a row.  Down to the right on the resulting form you'll find the "Add an implementer" option. Assuming one has access to only one "Organization" (and I've given support just one), clicking it gives a different look than one gets by looking at the actual form.  The latter is found under Step 1, view "List and Edit Implementers," by clicking "Add."
