Anyone have any experience using a Zoho CRM web form on a Wordpress site using Optimizepress 1.0 as the theme?

I am trying to insert a Zoho CRM web form onto an OptimizePress 1.0 lead capture page.

The data is not passing over as a lead into the Zoho CRM account.

Does anyone have any experience and tips for accomplishing this?
