I have a form Form A (see below screenshot) where there are two columns : Market and MVPD. Each row of this form is Unique 

I have another form where I need to auto fill the fields based on  selected drop down value and above form data . see below screenshot 

Example: When I select "M1" from "Market drop down field then I want to run a script that will fetch the related MVPD values from Form A . see below when I select Market as "M1" --

Could anyone please help me what script I should Use ?
