Can't add a lookup field.

I am on Windows 7 Ultimate 64-bit, using Internet Explorer 8.
 
I have two forms: one for tracking calls, and one for tracking issue types.  The issue type form has one no-dupes field in it.
 
Steps:
  • Go to the call form.
  • Drag the Lookup field to the form:
    • Change the label to Issue Type.
    • Import the "Issue" field from the issue type form.
    • Display it as a multi select field.
  • Click Done.  The field displays on the form.
  • Click Access This Application.

Error:  The field is not displayed.

* Click Edit This Application.

Error:  The field is not present at all.

I have allowed edit access to support.
