I need to create a form based on the below requirement, any assistance is greatly appreciated

As you can see from the below image i have all the customers stored using Customer Form, i need to create a new form where i pickup the customer from the customer table and have the action fields with radio buttons or checkboxs.

i can create a lookup field and individual action fields and the user can select individual customer and select the action fields, but its too cumbersome to do it for tens of customers every day.

What i want is to display the customer names as labels in the first column of all rows then i will have action fields as coulmn headings with a radio button or checkbox for the action in each column of the row, then user can select the values for all the customers and its related actions from a single page without going through it multiple times
