How would I make a form that looks like this form including custom placement of fields, buttons etc. and also tab controls just like the form I'm writing this posting in the form creator?

I'm assuming it's also made with the form creator.

Currently the form creator will only create very crude and basic looking forms that stretch across the entire web page.
