Creating a "table" form input

Hi,

Is there a way to create a table that a user would fill out?  For context, I'm trying to create a table that has "Cost Center", "Cost" and "Tactic ID" as the column headers and would like the user to be able to enter multiple lines (i.e. split the cost of a project across several cost centers.

Is there a way to do this?
