Duplicates/launching same application multiple times

I'm creating an application that I would like to launch to all students on campus. I would like to relaunch the SAME application every semester to update the students file on which classes they have taken. How can I do this? Also, how can I make sure that every time they fill out this application, their information will automatically update their previous report and not add a duplicate student with the same name?
