Email notifications activated...not receiving email notifications

Hi, I created a simple form that I'd like to use to capture data and then email out to a variety of email addresses. I've gone through the instructions and have done the Set Email Notification section. Everything looks fine, but when I go and test the form by submitting some info, I never receive an email.

Any thoughts why that is?
