form creation help
I'm trying to create a form
I want phone_type to be hidden on page load and I want it to show only on user input from phone_number field.
I want phone_type to show up when some one enters their phone number, but when I click save script I get an error on part of the form that I didn't even modify (see picture) what am I doing wrong. the code that I used I got from zoho website, http://help.creator.zoho.com/Hide-and-Show.html, number 3
