Something is corruped in my application

try to Edit form in Edit application mode:

ito_mu account - > EP_Attestation program, Select Systems  form

Form is not loading

I cannot even delete this form

I cannot create deluge script now for this application, and also when I try to view form definition from script mode, I get "Loading" forever. Please help me out here
