Function Insert into

Hi,

I have script which insert the data in to different form when condition happened, from last two days it stop working, i have gone through the script, it's fine no change has been done.

Please do the needful.

I have already allow access to support. my form name to_do_form (application penta_crm_10).

Script start from line no 115 to 141.
