How do I map contact details for more than one contact (forms to CRM integration)

How do I map contact details for more than one contact (forms to CRM integration)

I am currently finalising a membership application form on zoho forms and I have come across an issue. 
On the form participants can submit contact details for more than one person. We need these to be mapped and pushed through into the CRM as new records and to be also associated with the given Account name. Currently I have discovered that only one contact is being pushed through successfully, I am having to manually add the others.
Any help would be much appreciated!!
