can somebody help me with the form designs in zoho people?

actually am trying to design a table in a form that will contain some fixed text / description / contents under some headings. and then an option to enter some numeric value by the end user on respective contents. and in the end the total will be calculated.

for example

is it possible to design a table like the above in a form where the C1 & C2 contains some predefined values and texts and the end use has to just put the scores according to the particulars given????
