how to add record via URL (http)?

hi

there is website, which can send via http data from filled form

website send it as "http call":
http://my_site.com/?name=NAME&email=EMAIL@EMAIL.COM&phone=PHONE_NUMBER&lead_id=225298

http://my_site.com i can replace with http://creator.zoho.com/my_application/my_form/

how can tune my form so that it can record new entry when such "http call" from website is done?
