How to create an "Add New" button?

Hi

I have created a form to track repaired equipment. How to add a "Add New" button next to Submit and Reset, which could re-load the same data fields already in the form "n" number on times until the technician is done repairing all the machines assigned per machines per jobsite. Any ideas will be appreciated.
