Need 3 scripts added (paid job)

I need someone who is knowledgeable in adding scripts to a Zoho Creator form to add these criteria to my form.

Doesn't allow duplicate entries. Can't seem to do this based on the URL, which is what I need.
Records need to automatically drop off (be deleted) once they expire, based on a date.
Person submitting must put proper title to the URL or it will not submit.

Please let me know if anyone out there an edit my form to do these things.
