I have been trying to create a custom form, kind of a diagnosis tool for a medical website, that will react according to what people input.
I would like you to help me a little if possible with the basic/simplified template, and I should be able to work the rest by myself (hopefully!). I have tried to work it out by myself by looking at the help section, the support forums and by trying it but I just cant make it happen.
The form should help user to find common outcomes and treatments for their injuries according to all the cases we will add to the database. But first we have to identify what is the user's injury.
All the fields would be dropdowns that would progressively lead the user to the appropriate page at the end of it:
Lets say the first field is:
1- Injury Type: Work accident or Road traffic accident
If the user selects "Road Traffic Accident" he/she would then be prompted to a second field (which would be different if it is a "Work Accident"):
2-Passenger position: Driver Seat, Front Passenger Seat or Back seat.
Once the user select the "Passenger Position", it will then be taken to the third field:
3-Diagnosis: Consequences, Outcomes, Treatment
Now once selecting an option in field3, each of these options would take the user to a custom page that will be generated according to the three previous answers.
So say if the user had a "road traffic accident" and was in the "driver seat" and wanted to see the "outcome" of this, it would be taken to the relevant page.
I hope this is clear enough, if someone could help me with this basic idea it would be greatly appreciated as I could simply replicate this process accordingly and create a more comprehensive Zoho Form.
