I have created a form and need to populate a lookup field based on a previous selection.

This is the example.

I have a form that has client information.  The client information form has 5 statements provided by the client.

In the evaluation form, we select the cleint from a lookup field.  The next field needs to look up the 5 statement from the client information form based on the client that was selected.

Is this possible?
