Problem submitting edited forms linked to by URL

My users have reported and I have confirmed, that when a record in a certain form is accessed by a URL for editing, and the Submit button is pressed, the "Please Wait" message appears and seems to hang.  The record is never updated with the edits.

Sometimes the URL just leads to a blank white screen, i.e. the editing form does not appear.   Other times the editing form appears, but cannot successfully submit.  And then, a few times, it will all work as intended -- i.e. edits are made and submitting is successful.   This has been observed many times.
