I would kindly like to get help in making a search interface.

I have already created the stateless form with the search button and also the html view. 

However, am having difficulties in writing the script. 

Ideally the search form has 6 fields which can be used to search,I would like that an input into any of the fields gives a result. 

Any ideas and help will be highly appreciated.
