Settings not saving

Hello,

I have started on a new revision to one of my applications. "Tool Tracking v2" In form "RFC 5x5" there are two fields that I can not get my desired settings to save.

First field: Description: Unable to save the Initial Value, or the Hide this field to others option.
Second field: Part Number:Unable to save the Initial Value, or the Hide this field to others option.

This feature is working in other forms with no problems. Edit access is already granted.

Please Advise.
