Stateless Forms, Need TA Quickly, Willing to pay via paypal

I am working with a client DB and need to track various level of information over time.  The central nucleus is the client, so all information will loop back to them.  Different forms will be used at different times to enter information about the client (i.e. a patient, and all records relating to each visit will contain different fields, but we want to be able to bring up a view that has the client and shows all information about the client pulled from different fields.)

I know this is possible but I do not have scripting experience and need an expert in Stateless Forms.
