Tasks List Table with Link to Forms and Records

Tasks List Table with Link to Forms and Records

I am trying out Creator and I want to create a task list for employees that has a field which links to a form and pulls up a specific record for the employee to update.  How can I do that?

Example Table:

Project
Step
Task Name
Form Link (with new record or dynamic new record)
Status
Next Step Approved
Next Step Rejected
