Hello, I have a problem with connection of two variables in one dropdown menu.

In one form I have two fields: "name" and "email" which You input in textboxes,

in next form i would like to have a dropdown with values in form like: name(email)

and after picking one of values from dropbox i would like to send a message to the chosen mail.
