Where form results open

Where form results open

My form results are opening on top of the form itself and I am not taken to the next URL. My form is at http://jaxareagolfassn.com/Championships/2012/ClubChampionship/2012ClubChampEntryForm.html but I want the user to be taken to http://jaxareagolfassn.com/Championships/2012/ClubChampionship/ClubChampReservations.htm.

What am I doing wrong?
