A form integration field fails to load on some user's browsers, and the html renders incorrectly.

This is a new problem on a from that is unchanged,

This is happening on external customer's machines. I Assist'ed into each computer, and replicated the problem in Edge and Chrome (both Chrome, I know). Browsers are current on those machines. 

Any ideas on where I should look to fix this? It seems not to be my problem or Zoho's, but, of course, I need to find a solution.
