Adding candidates via another web form

Hi,

I am interested in setting up a web form for Recruitment Agencies to be able apply submit candidates. The form would need to be able to add candidate data such as:

First name
Last name
Email Address
and other candidate data fields

In addition it would need to be able to upload a candidate's CV document against their record and associate them with a job.

The key feature of this form apart from the adding of data such as Candidate Name, Candidate Email etc would be the ability to update the SOURCE field which currently lists applications via the web as EMBED. I would want to be able to add a 4 digit reference number into this field.

Is this possible?
