Any way to transform a submission from Zoho form to an attachemtn in CRM??

We have a form created in Zoho Form, we need this one to go to CRM, but not as an entry on any Module, instead we need the information taken from the form to be an attachment and put it on any Lead, Contact or Account on CRM. For example.

By selecting a name field, its going to a certain account, not as an entry, but as an attachment. 

Is it possible??
