Google tag manager not triggering

I followed the guide to set up the conversion tracking through google tag manager.

The conversion through Zoho Forms is still not triggering as it should.

We got 2 conversion tracked and this is the link where it happened

It seems to be just the test link.

What can we do to track all the conversions through GTM?
