How do you export each form entry as a separate PDF?

How do you export each form entry as a separate PDF?

Say I have 5 form entries how do I export each entry as a different PDF so that I will have 5 PDFs. I have tried to find a way of doing this but have been unsuccessful. I have found how to export all 5 entries as one PDF however I need desperate help on how to save each individual entry as a separate PDF? Thanks in advance for the advice and assistance.
