Is it possible to build a custom field that combines some of the different aspects of the basic fields into one line (or field)? 

Such as... We would like a field that combine 2 questions with checkboxes as well as have a text box to be able to add a note about the selections and upload a photo. 

I realize we can do this by adding additional fields below one another but it makes the form very very long. 

We have a 96 point checklist that our technicians perform and each one of the 96 items contains 

1. Checkbox of Item Condition

2/ Whether it was repaired or not

3. Details of the item or service

4. Photo of item
