Job application randomly not showing fields

Job application fields appear to be correct and complete when I look at them through Zoho and go to the application directly. However, it seems like every 1 out of 10 candidates ends up missing a mandatory question (e.g., they don't upload their resume or cover letter). I've checked and double checked the field settings and the application itself and everything shows and is mandatory. So why is this randomly occuring?
