how can i create a report with the following colums: 

Date | Potential | Account | Subject | Description  

I want to be able to run a weekly report which shows me every "Log a Call" that has been clicked by a sales rep. 

Lastly, I'd like to build a graph of the number of "Log a Call" activities occurred per wk.
