QuickBooks – Application Certificate popup window

I am following the instructions to Add Zoho CRM Application to QuickBooks Web Connector (QBWC)  http://www.zoho.com/crm/help/quickbooks/install-quickbooks-web-connector.html#Add_Zoho_CRM_Application_to_QuickBooks_Web_Connector_(QBWC); however, I never get the Application Certificate popup window and cannot allow the application to read and modify this company file. 

I have deleted the file; deleted the QuickBooks program and reinstalled and everything else I can think of, but never get this screen unless I do their test file which works perfectly.
