How to replicate this ?

basically i created a simple note.

i took pictures directly using the camera app.

initially it was able to display all those 3 pictures.

suddenly after the sync process, all those 3 pictures were not sync successfully to my ipad.

it showed the broken images.

i have set the option "Save to Gallery" to On , but i cannot find the pictures inside my gallery  , so basically now , all the pictures are gone.
