Using Forms in Facebook tabs?

Has anyone successfully embedded their form in a Facebook custom tab? FB treats tab content as an iframe, so all you should need is a secure URL to feed into the tab. However, the form isn't parsing and all I get is a blank page (I can't seem to view the source of the frame to see what's loading).

Any advice is greatly appreciated! Let me know if you need me to fill in any info.
