Add entry UI problem in FireFox

There's a problem with the "view" mode GUI in FireFox. If you click on Add, you'll be given a window that looks like two of the same window stacked on top of each other vertically. However the first one is empty, pushing the 2nd window way down in the bottom of the screen making the Update and Cancel button inaccessible. This problem doesn't exist in IE.
