Aplication dissapeared

I've created a form application that you can see here

Sometimes works and sometimes is asking to login in to zoho to view the application and sometimes it says doesnt exist.

Now if I login into my account , the form is not there and i cannot view any of my other form neither, I really need this application working.

I'll appreciate any help.
