Attaching a File Bug

When I submit a form and upload a file using the file field everything is ok. Then I immediately submit the same form again with different information and a different file, the original file is uploaded. I've tested this several times and even double checked the file path of the file.

I can't prove this has happened to others using the form so I wonder if this is a browser problem on my computer?
