cannot create custom filter

I've created a bunch of custom filters for one of my views. But I can no longer create any more. When I try to create a custom filter, no fields show up to input criteria. Also, I hit "Save" but nothing happens.

Another problem is that the custom filters seem to always be "Match Any". Even though I set it to "Match All".
