Cannot get form to update via e-mail

Good Morning all
Getting my form to update via e-mail is giving me FITS!
Sometimes it works other times nothing.

Does anyone have any ideas?
Is there a known bug with this?

I have made the form public to avoid any security issues... although at some point i would like it to accept e-mail from only 1 address.

I only copy the template and edit it per the instructions.
have tried html and text thinking that i am missing something.

Do these e-mails go somewhere that i can troubleshoot?
Are they not arriving at the address used for the form?
is there an error log that i can review?
Please give me something to work with.

Why will some records show and not others?

I am REALLY confused and about ready to give up... this has made for a very long week.
