The app is hosted under her name, but when I log in as a user everything works just fine.

When she logs in,  the form "Purchase Order" doesn't show up when you click the "Create New Purchase Order" heading at the top. 

I've logged on under her login to see this. When I hit edit, then access this application, the form loads correctly, but if I navigate away from it, then back, it doesn't show up again.

Please help.
