Question 1: Who was involved in the incident. To achieve this I have a lookup field (Names) being displayed as a multi-select field. 

Question 2: For each of the people involved, were they properly licensed. 

There is not a definite number of names that will be selected each time. I would like to have a workflow set up where I have a response option for question 2 for each of the names selected in question 2. For example: 

Who was involved in the incident: 

Jill

Bob

Fred

Was/Is Jill properly licensed?

Was/Is Bob properly licensed?

Was/Is Fred properly licensed?

Seems like this should be possible, but I cannot figure out the right way to do it. Any assistance would be greatly appreciated.
