Creating script to automatically display record in field

Hi,

I've been trying unsuccessfully to automatically populate a field with records from another form.

Problem:

I would like to automatically show the ID for a facility when the corresponding NAME is selected from the preceding drop down field.

I posted earlier but received no response.

Can anyone help?
