Can someone help me to achieve this process?

From the Potential view I want to add a custom button to schedule a new appointment on a existing Potential.

I want the user to be directed to the scheduling link pop up with Potential Name, Email and Phone pre-populated in the form. User would them complete the remaining required fields.
