Dashboard issue when adding Quick Create - New Request

Dashboard issue when adding Quick Create - New Request

I found that my dashboard has an issue when adding Quick Create - New Request. It does not allow to save the contents to the database, and comes up with an error:

Error occured while executing on click script.

Please let me know where I could be troubleshooting this issues.  I haven't found the original form for this embed.
