Data Save Problem - My Last and First Name Saves to Every Ne

My Last Name (McGee) and First Name (Patrick) is over-writing and saving as a default value into the last and first name fields of any new record I create or existing record I update.

There is no default data setup for those two fields. How did this happen and how do I clear it? Every record I open and try and update is overwritten with my Last and First Name! I've tried working on the database from different browsers and different machines with the same result, so I know it's not my browser.
