Database form is broken - can't edit or delete.

I have a form that has broken. I created it OK from an XLS upload, but when I tried to change one field, the form stopped displaying. I suspect my change was not accepted properly and now I can't delete the form. How do I delete the form ? I was trying to change the "Year" field title to "Season" and the max digits to 4.
